Athletes and coaches must have a current membership card at the State Championships. If your USAW card has been destroyed or lost, you must purchase a new one at a cost of $35.00 online, plus fees.
If you move to a new address after purchasing a card, send a change of address form to the State office to continue your USAW newspaper subscription.
No Roster will be accepted at events—all purchases must be online.

Athlete and coach memberships carry a Secondary Sports Accident Insurance policy in effect at a sanctioned event or chartered club practice only. There is a $500.00 deductible and an 80/20% coverage up to $2,000.00 out of pocket & then will be 100% coverage (not including the $500 deductible) in a benefit period which runs from September 1 to August 31.
Club Charters carry a $2,000,000 liability policy effective when chartered through August 31st. By insuring the site with the form Adding Certificate Holder as Additional Insured and accompanied with the Charter application, they are also insured with the $2,000,000 liability policy.
Sanctioned events are covered by a $2,000,000 liability policy effective the date of the specific event. The site can also be insured by using the form Adding Certificate Holder as Additional Insured along with the Sanction Event form.
